Appointment Information & New Clients 

Your business is appreciated. In order to best serve you, please be aware of the following guidelines. 

 

Booking

All services are scheduled by appointment only. Pure Bloom Skincare does not accept walk-ins.

 

Prior to Appointment 

Please print & fill out the following form. If you are unable, a form will be available at the studio for you to fill out:

 Client Consultation Form

Arrival Time/Late arrivals

Please aim to arrive 10 minutes before your scheduled appointment time. If you are more than 10 minutes late to your appointment, this will result in your service being rescheduled for another date. This is to insure the client booked after you is not affected by your late arrival.

Cancellations 

Cant make it? Please call within 24 hours to cancel and reschedule your appointment. Everyones time is valuable, please be considerate. 

 

Youth

Any client that is under the age of 18 must be accompanied by an adult, who may then sign the client consultation form before the service.

 

Sickness or family emergency 

If you or another person in your household, has an infectious or contagious illness, please contact me as soon as possible to reschedule your appointment for a later date. There is no penalty in this case, everyones health & safety is important. 

Handicap Accesible?

The building that Pure Bloom Skincare is located in, is NOT handicap accessible.  

Gratuity

Gratuity is greatly appreciated, as it is not included in treatment prices.
